  • Hearing screening

  • Health education and anticipatory guidance
  • Laboratory tests for:
    • Anemia
    • Blood lead
    • Tuberculosis
    • Urine abnormalities
    • Sexually transmitted diseases
    • Other problems as needed

Referrals


At the time of the health assessment, CHDP providers make referrals to:
  • Women, Infants, and Children (WIC) Nutrition Program for children to age five
  • Dentists for preventative or restorative care
  • Medical specialists as necessary
  • Mental health providers as necessary
