What services does CHDP provide?
CHDP provides periodic preventive health assessments and services available to eligible children. CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. At the time of the health assessment, CHDP providers make referrals to:
Women, Infants, and Children (WIC) nutrition program for children to age five; Dentists for preventative or restorative care, and Medical specialists as necessary; Mental health providers as necessary.
